Annual Student Forms
Complete Required Annual Forms by August 15
Orleans Southwest Supervisory Union (OSSU) uses PowerSchool to collect important information from parents and guardians, including annually required student forms. It is required that parents/guardians verify enrollment for their child(ren) by completing the annual registration and health forms for 2025-2026 located within the PowerSchool Parent Portal. Please complete, if possible, by August 15, 2025. This is a step all parents and guardians – returning or new – must take before the beginning of each school year so we have current, updated information for your child. School begins on August 25, 2025.
At least one parent or guardian must have a PowerSchool Parent Portal account with their child(ren) linked to it to complete the required forms. All parents and guardians should have received an email from the district with instructions and codes to create an account and link their child(ren) if they haven’t already. Check your email for this important communication! If you cannot locate this email, and you do not have a Parent Portal account, contact your school’s administrative assistant or registrar as soon as possible. For parents or guardians with a Parent Portal account, first, go to: https://ossu35.powerschool.com/public/ Click on “Sign In” and enter your Username and Password:
If you do not remember your username or password, click on “Forgot Username or Password?” to reset, then login.

Check to make sure that all of your students are linked to your Parent Portal account by reviewing student names in the blue bar at the top of the page. If you do not see all of your students in the blue bar, please contact your school’s administrative assistant or registrar for the Access ID and Password for any missing student (s) before proceeding. Click on Forms, which takes you to the Annually Required Forms tab. To complete the three forms, you will need for each child:
-Immunization records; Click here to view required immunizations.
-Proof of Residency (only if your physical address has changed)
-Optionally, you can provide insurance information
